Abstract Aim The aim of this retrospective study was to analyse the discrepancy between preoperative MRI staging and post-operative histopathology rectal cancer patients who had primary surgery. Methods A cohort analysed records 47 with histology confirmed over period 5 years from 2017 2022. Data collected hospital database. Results Forty-seven were evaluated. mean age presentation 72 years. Out 65.95% (n=31/47) men; 34.05 % (n=16/47) women. Discrepancy in T N found 44% (n=21/47) 49% (n=22/45) respectively. Two inconclusive on MRI. In stage discrepancy, 86 ( n=18/21) upstaged 14% (n=3/21) downstaged. 73%(n=16/22) 27% (n=6/22) Conclusion There significant our study. has critical effects management, outcomes, survival rates patients. Appropriate accurate clinical radiological enables multi-disciplinary teams plan optimal management approaches.
